  3. Perjury statement: A statement under penalty of perjury that you have a good‑faith belief the material was removed or disabled as a result of mistake or misidentification.
  4. Your contact and jurisdiction consent: Your name, address, and phone, and a statement that you consent to the jurisdiction of the Federal District Court for your address (or any district in which the service provider may be found) and that you will accept service of process from the person who filed the original notice or their agent.

Send counter‑notifications to: [email protected]. After we receive a valid counter‑notification, we will forward it to the original complainant. Unless we receive notice that the complainant has filed a court action seeking to restrain the allegedly infringing activity, we may restore the material within 10–14 business days of forwarding the counter‑notification.
